Structural Change of Total Health Expenditure Caused by the Reform for Separation between Prescribing and Dispensing of Drugs in Korea

Abstract
Total health expenditures of the years 1998-2001 according to the manual "System of Health Account" by OECD were estimated on the basis of various data. Data from both Urban Household Survey by National Statistical Office and National Health and Nutritional Survey by the Ministry of Health and Welfare in Korea were used to estimate expenditure
that are financed by out-of-pocket payment of the patients, while national health insurance data etc. were used for the estimation of health expenditure that is financed by public funding sources. A big reform for the separation between prescribing and dispensing of drugs (SPD reform) was introduced in Korea in the latter part of 2000. This reform was expected to have influenced overall aspects of Korean health care system. The estimation results of this study show what follows.
1. SPD reform augmented the public financing coverage in Korea. Share of public financing in total health expenditure increased from 48.7% to 54.7% for the three years between 1998 and 2001.
2. SPD reform strengthened unusual mix of health expenditure by mode of production in Korea compared with other OECD countries. Share of inpatient care in current health expenditure decreased from 31.4% to 23.7%, while share of outpatient care increased from 33.5% to 42.4% and share of drugs and other medical goods, from 26.0% to 28.0%.
